LETTER OF DISCIPLINARY ACTION

Friday, April 4, 2003

Dear Ms. _____,

_____ has not been on task this week. He received detention on
Wednesday and Thursday respectively from Mr. _____ and Mrs. _____ for classroom disruptions. He did not show up to serve his detention today. Furthermore, he continued to disrupt the physical education class with similar behavior.

I have discussed his behavior and the consequence with him, which will be in school alternate placement (ISAP) for Monday, April 7. He was respectful and accepting when I discussed the importance of his classroom behavior in relationship to his education and that of his classmates. I appreciate that, but do expect him work extraordinarily hard next week to make up for the instructional time he lost this week.

LETTER OF DISCIPLINE AND REFERRAL


February 18, 2003

Dear Mr. and Mrs. _________________,

Mrs. _______ was not at school today. The _____ class was supervised by
_____ and Ms. _____. They report that although _____ was a little
intolerant of others, he did not have a bad day. Unfortunately at 2 PM,
the class was preparing to have physical education class outdoors. Some
students were skating. Others simply planned to play. _____ wanted to
go skating, but did not have his snow pants. He did not respond to the
situation well. He immediately stated, "I'd rather just go home and
commit suicide." Mrs. _____ and Ms. _____ encouraged him to go ahead
and get dressed. He went outside without further incident.

As a substitute, Ms. _____ did not feel comfortable addressing the
situation any further and requested I spend some time with him. I did
so. _____ expressed he is very sad and just doesn't have anything he is
interested in doing or anyone he is interested in being with. He
continued that he would rather freeze in an ice block than go on
living. I reassured him how so many people at home and school care for
him.

Although I told him he cannot express thoughts like that in front of his
classmates, I told him to let an adult know anytime he was experiencing
those kinds of feelings. We take this situation and his thoughts very
seriously. Please keep us aware of any behaviors or actions we should
be mindful of.

-----------------------------------------------------------------------------LETTER RE: ATTENDANCE

February 1, 2003

Dear Ms. _____,

We are very concerned about _____ attendance. She has missed
twenty-five days the first semester of school. Alaska State Law (A.S.
14.30.020) states five days of unlawful absence constitutes a violation
of state law obligating the school and you, the parents, to develop a
plan for improving Tina's attendance.

_____ is struggling academically and socially, we believe in part due to
her attendance. It is very difficult to catch up on new concepts and
materials after missing several days of school. Likewise it is
difficult to build and maintain personal relationships when she is not
consistently present.

According to Copper River School District Elementary Handbook students
who acquire thirty days in a school year may be recommended for
retention in their current grade level. Tina is repeating kindergarten,
which rises our concern level.

We recently left a message with you on your recorder. We would like to
take this opportunity to request a meeting to create a plan of action to
make the most of _____ school experience.

LETTER OF DISCIPLINARY ACTION


February 7, 2003

Dear _________,

As we discussed on the phone, _____ had a difficult time at lunch. He spent the remainder of today in school suspension and will continue to serve Monday also. He took full responsibility for his actions.  He feels his behavior and irritability are due to lack of sleep and perhaps medication problems.

Thank you for your support and please do let us know if there are any specific behaviors or signs we should attend to at school. Please feel free to contact us if you have any concerns.
