Before attending practice all skiers must complete and turn in to the activities office:

1. Physical Exam Form

2. Sport/Athletic Participation Form

3. Activities Fee paid to South Activities Office $160 (waiver available)

4. Watch the Play For Keeps Video

5. Sign the Play For Keeps Acknowledgement Form

 

You also need to complete and turn in to a coach:

1. Your contact information sheet.

2. Your $40 booster fee

3. Your Team Contract

4. The yellow copy of the Athletic Participation Form
