Dear Parents,

After school basketball has begun. This is a great opportunity to get exercise and participate in a team sport. It is necessary for all students have a current physical on file prior to attending.

I will provide transportation for students to attend the program, however you will be responsible for picking your child up. Please see the attached schedule for practice days and games.

Please indicate below who has permission to your child up daily. Also provide an emergency contact name and number in the event of an emergency in which you cannot be reached.
