Bodenburg Butte and Moose Creek Field Trip

PARENTAL CONSENT FORM

Dear Parent:

_____________________________________________, a student in Mr. Holm's geology classes, is being offered the opportunity to participate in the activity described below. The trip will be supervised by school personnel and parents. Should a medical emergency result from participation in this activity, you need to authorize medical treatment for your son or daughter. If you consent to these conditions of participation, please sign your name in the space provided and have this consent form returned to Mr. Holm in room H104 no later than 2 PM Friday, April 27, 2001.

Students will take part in a field trip to study the glaciation of the Matanuska Valley, and to hunt for fossils along the stream banks of Moose Creek. A school buss will leave Service High School at 9:00 AM Sharp, Saturday, April 28, 2001, and take students to the base of Bodenburg Butte. After a hike to the top of the butte, busses will take students to Palmer for lunch. Students may purchase lunch at McDonald's, Carrs, Subway, or they can eat a lunch brought from home. Busses will then proceed to Moose Creek, north of Palmer to hunt for fossils. We will attempt to return to Service High School by 3:00 PM. Additional information about the field trip is outlined on a separate flyer.

There will be a charge of $4 per student wishing to take part in the field trip to help pay the cost for the school busses and parking at the base of the butte. This fee must be paid to Mr. Holm no later that Friday April 27, 2001, either in cash or by check, made out to Service High School.
